How does ablative erbium laser work for wrinkle removal and skin tightening?
The Erbium Yag laser of 2940 nm produces a non-selective cold ablation of the entire surface of the treated region, layer by layer.
The laser beam leads to vaporization of the epithelium because its wavelength causes the laser energy to be absorbed in the water molecules of all the cells of the superficial layer of the skin. The target chromophore is a water molecule.
Mechanism of laser skin rejuvenation
- The laser beam produces a controlled burn while preserving the follicular epithelium that ensures re-epithelialization:
- THERAPEUTIC effect is achieved by simultaneous targeting of various irregularities in the skin structure. in the deep, middle and surface structures of the dermis and epidermis
- and thus the tissue reaction to thermal heating is initiated.
- Cytokines triggered by thermal tissue damage provide a signal to initiate the wound healing mechanism, which activates epithelial cells and fibroblast-building cells, resulting in the restructuring of all parts of the epidermis and dermis.
- Improving the quality and appearance of the skin depends on the precision of the treatment of precisely defined layers of the skin.

Ablative Erbium and CO2 lasers target the same skin layers, namely the epidermis and the papillary part of the dermis (the superficial layer of the dermis).
The great advantage of Erbium compared to the CO2 laser is the possibility of cold ablation without excessive thermal effect on the dermis, which significantly reduces the possibility of complications in the form of post-inflammatory hyperpigmentation.
The CO2 laser targets the epidermis and deeper parts of the dermis and causes a stronger response of the wound healing mechanism because it delivers a greater amount of heat to the dermis, but has a significantly increased risk of post-inflammatory hyperpigmentation and requires a significantly longer recovery.
Compared to the CO2 ablative laser, the Erbium laser is more precise, as shown by the results of clinical studies.
By measuring changes in viscoelasticity of the skin immediately after treatment and histological research, it was concluded that:
- ER:Yag laser more efficient and precise in derma ablation than CO2 laser, and yes
- CO2 has a deeper thermal effect, which does not contribute to a better result, but only increases the risk of side effects in the form of spots.
- Given that changes in skin viscoelasticity are correlated with the amount of delivered energy, the Erbium laser shows a significant advantage over CO2.
- This is the main reason why we replaced the CO2 laser with an Erbium laser 15 years ago. And this has been scientifically proven by biopsies after 2 days of treatment. The CO2 laser showed a higher level of inflammation in the lower parts of the dermis, which suggests a deeper thermal effect and a greater possibility of complications in the form of hyperpigmentation or hypopigmentation.

Side effects - our experience
Very low incidence of side effects (mild and transient).
- Prolonged pain in the form of burning and stinging – in a mild degree lasting 24h – 48h in less than 2% of cases.
- Swelling, tension and redness of the skin – in a mild degree lasting 48 hours in less than 5% of cases.
- Reactions are short-lived and usually resolve spontaneously within 24 to 48 hours.
We have only had a few patients in the last 15 years of working with ablative lasers when, due to pronounced erythema – redness and edema – swelling that was present for more than 48 hours, we had to include local and systemic therapy, which very effectively accelerated recovery and eliminated side effects.
